Abstracts

PURPOSE: To conduct a systematic review and meta-analysis comparing the effect of excimer laser trabeculostomy (ELT) ab interno as a stand-alone or combined with phacoemulsification (Phaco-ELT). METHODS: Following PRISMA Guidelines, PubMed and EMBASE were searched, resulting in 14 eligible articles. Endpoints included absolute reduction in glaucoma medication, intraocular pressure (IOP) and visual acuity (VA) at 2 years. RESULTS: Data from 1,549 eyes was evaluated. At 2 years, ELT reduced glaucoma medications by 1.26 (95%CI: 0.74, 1.78; n=95) and Phaco-ELT 0.58 (95%CI: 0.33, 0.83; n=130). IOP decreased by 7.10 mmHg (95%CI:6.25, 7.95; n=105) for ELT and 6.72 mmHg (95%CI: 3.01, 10.42; n=130) for Phaco-ELT. VA improved in both procedures with few complications or subsequent surgeries reported. Complications in phaco-ELT at 1 year were 4.09% (95%CI: 1.96%, 8.34%) and subsequent surgeries at 1 year were 5.55% (95%CI: 2.45%, 12.09%). CONCLUSION: These findings support the efficacy and safety of ELT as a minimally invasive glaucoma surgical procedure, reducing IOP and medication burden while improving visual outcomes, whether used alone or with phacoemulsification.
